We will need a referral from your primary care or treating physician to schedule you for an appointment. If your insurance requires authorization, we will also need a referral authorization from your insurance company. We do accept self referrals, but the insurance authorization is still necessary when your insurance company requires one.
Keep in mind that if you are scheduling your first appointment with us, you may need to come in to the office at least 20 minutes prior to your scheduled appointment time to complete paperwork. To reduce your wait time, we may email or fax the required forms to you so that you may fill them out at your leisure and bring them in with you to your visit or get them to the office ahead of time.
We try very hard to see you at your scheduled time, however, it is important that our physicians respond to medical situations that may require extra attention. Should this occur and affect your appointment time, we ask for your patience as we do our best to accommodate everyone. We thank you for your understanding.
